De film is losjes gebaseerd op een kort verhaal uit 1926 van Jun'ichirō Tanizaki en begint als een kunstenaar en een jonge vrouw in de wachtkamer van een tandarts. Hoewel hij zich tot de vrouw aangetrokken voelt, zegt hij niets tegen haar. Ze bevinden zich later in dezelfde onderzoekskamer. Wanneer hij verdoofd wordt, begint hij zich een reeks scènes voor te stellen waarin de vrouw verschillende vormen van seksueel misbruik ondergaat, waaronder verkrachting en marteling. Wanneer de kunstenaar herstelt van de verdoving, vindt hij aanwijzingen waaruit blijkt dat hij mogelijk niet aan het hallucineren was.
